My wife, a teacher, failed her PADI OW exam by two questions. When she talked to the instructor about her results, he told her to take the exam again and not too think too much about the answers and not to read anything into the questions; she passed with 100%.

Just finished the 2 last exams Deco theory and environnement.

I kept an average 18/20 for the 5 test. It was just tooooo easy for a "professionnal" level.

For the prepared person, they may appear easy, but they are not. Failures at the Pro level most often occur on the Theory exams, especially at the IE.
