Is this a function of someone setting the lever height too high? Or is this just something you get from them being old?

It happens with age. The diaphragm is worn out. As the rigid disk in the center of the diaphragm begins to "cup", it will impact the velocity of the purge and will also make it increasingly difficult to properly set the inhalation effort without raising the lever beyond the nominal case-level setting.

Why I have never seen this happen with other regs? Did Apeks just make the decision to go with a lighter plastic bit in the diaphragm for performance reasons?

(Do you know where I can buy ten of those?)
