I have the Axiom I3 and really enjoy it. Ex Navy diver that dives in PNW with dry suit most of the time. Super comfortable, easy to adjust air. Fits just as tight or loose as I want it for any given dive condition. Also can go from wet suit to drysuit, warm weather to cold and bunch of layers without having to adjust anything. Naysayers talk about bumping and loosing air, being a catching hazard, being complicated to explain, and more parts.
Bumping - Yep, once in a while I bump it and a little air goes in. Never a big deal. Like most divers, I don't keep my valve open until I am ready to don it, sure wouldn't keep it open if it was in a pile with a bunch of other gear. That is unsafe.
Catching hazard - I always find this funny because the more minimalist the BCD, the more stuff gets hung from it. Some divers look like Xmas trees because they have no place to put stuff. All vital stuff but it has to go somewhere. Talk about catching hazards. The regular inflator hose seems to me just as much a catching hazard, and it floats around. On the Axiom model I have, the right pocket can be dropped down and made into a large pocket. Very nice when I find something I want to collect. Not huge, but also no need to hang a bag for most dives. Backup light tucks into another pocket. Two danglees out of the way. That and dry suit pockets makes me much less of a catching hazard that the minimalists.
More complicated to explain- Yes it is different, but not hard. I put it in same category as dump weights. Not all work the same so you have to talk to your buddy and remember. On the I3, the discussion is easy. "here is the lever, up makes you go up, down makes you go down." Frankly in a rescue situation the I3 would be easier to manage bouyancy if necessary. I have been the rescue dummy during rescue classes, so know this from experience.
More parts - yes it does, which increases the risk of failure. Like others say, keep it clean and keep it serviced is important. Also like others say, failures happen with all BCDs. However, if I was travelling often to really remote places would this be my first choice? Maybe not. I usually use a travel BC that is really standard or would opt for a simple BP/Wing. And besides, the vast majority of locations do have rental BCDs in a pinch so you aren't going to loose your whole trip.
Regarding the last two, complicated and more parts, interesting that the I3 gets a rap, but rebreathers and side mounts are fine.
