The one in four isnt really accurate, in some cases its totally asymptomatic in diving and depends on race group and so on.

PFO test is a fairly unpleasant invasive test involving injections and doppler. Its really not something i'd recommend to many people.

The link between PFO and DCS is also not that clear cut. Yes some large hole can cause the issue but in many cases it causes nothing at all.
The PFO/DCS issue is far from black and white. The Migriane/PFO issue is even less clear.
