What happens if an Australian gets OW certified on vacation elsewhere? Can he then dive back home without required medical clearance?

I figure a lot of tourists come dive that way.

Yep. Same home town as OP. My wife and I were certified overseas and can dive at home with no complications or extra paperwork. When we took our AOW locally late last year we weren't asked for anything. My wife has a medical condition that can be a contraindication for diving, so we sought full clearance and expert opinion for our own sake, but at no time was it forced on us.
