Intova torch switch failure

I have two units of the Intova Ultra Nova 2 purchased a year ago: http://www.caveadventurers.com/images/stories/ultranova3.jpg

One of them had a mysterious switch failure today, which I confirmed by swapping one of the torch's rear cap with the other. The switch is at the rear cap and is a magnetic button style switch.

The torches have not been underwater for 6 months, and were working when I checked them yesterday. I discovered the problem after I recharged and replaced the batteries.

I also noticed that the failed switch would briefly blip the LED as I screw in the cap, while the working one doesn't.

Is this a common problem that happens to switches of this kind?
 
The pic below is the inside of a flashlight tailcap with a switch.

Sometimes the switch parts get unscrewed and the switch doesn't work right. When that happens I take my pointed tweezers, insert them in those holes and tighten it back up turning clockwise.

Is it possible that you have anything loose like this in the tailcap?

Yes! It rattles compared to the working tailcap, so I screwed it back and it works fine now. It's still a puzzle how it came loose, but at least I know what to look for next time.
