Name labels for regulators and BCDs

At Puerto Galera this weekend, I saw a regulator and BCD with labels showing the diver down symbol and the owner's name. I thought that was a great idea, but was heading out on a dive and the owner was not around.

They looked to be about .75" x 1" or a little smaller, thin, probably plastic, and obviously waterproof.

Any idea where to get these?
