I am a VIP inspector. Here are the guidelines: I will always clean the valves and put new oxygen comp o-rings on with crystolube. During the inspection process, I not only inspect with a light that goes down into the tank, but also with a fluorescent lamp that goes in. This lamp will help identify any deposits that I consider to be hazardous to O2 fills.
Prior to performing the VIP I ask the owner a series of questions to determine if he has done anything that might contaminate the tank.
Finally, during the inspection process I must use absolutely clean tools to ensure no contamination. If for any reason I suspect contamination, I'll fail the inspection and require an O2 cleaning to put a Nitrox sticker on it.
