what goes on line 15 (identification number of student enrolled in course)? My instructor left that line blank. I did not realize it till I got back from my vacation. I did the peak performace bouyancy speciality course in Cozumel last week

and can I use a photo that is on file with PADI or do I need a new photo

For the life of me I don't recall "line 15". I don't have a PIC in front of me. I do know that I have never put a number on a PIC card, so I have never filled in a "line 15".

No, at your level of training you will have to submit a photograph with the PIC - put your name on the back of the photo and slip it into the sleeve.

Not sure if this will help, but on PADI Americas PICs, I have never found out what that # is for. I have never filled it out....

It would be safer for you to send in another Picture with the PIC unless you want to call PADI and coordinate using the one on file(assuming it is digital)......

Anyway dont worry about the # that was a recent change to the pics(a couple of years), and I am assuming it is for future use.
