Second stage recall?

What's with the recall of all models, save flight, of previously unserviced second stages purchased prior to July 2008?reason cited due to assembly error. Care to advise what exactly the assembly error was? Any additional info you can provide would be appreciated. Thank you.

You are referring to a voluntary recall notice from 4 1/2 years ago (June 2008) where Apeks was concerned whether or not any regulators got into the field that may have been missing their 2nd stage diaphragm retainers. As it turns out, there were no reports of any regs missing this part. Apeks was just being cautious.
