Is the card fee $10 and then the dives are extra or is it all inclusive?

What are the prerequisites? OW, AOW, Rescue, CPR/First Aid, etc.?

100 = silver
500= gold
1000= plantium

after 1000 and plantium card. You have to pay $200 a year for insurance. Where you will not be countible as a scubaboard, if others try to sue you.....
