How about an Android app that can:
* download dives via bluetooth from BT-enabled computers
* sync with the online log

why bluetooth? why not directly from the cloud?

It is interesting to look at another divers template and visualize what their typical diving is. I don't want to take-away anything from what you have since you obviously tailored it to your needs. For the benefit of other doing the same here are some twists one who is mainly a New England Shore Diver may add;

Dive numbers (lifetime and annual tally's)
Dive company through refreshments don't exist in that context so they wouldn't appear for me. (log as free form notes hen traveling)
Fees for park entry / parking are occasional, no tips involved
Conditions include tide, H+1 as an example of 1 hour past high tide at entry.
Add Drysuit and garments (all the others still apply!)
Regulator used
BC used
Specific cylinder AL80, LP72, MP72, HP80, HP100
Weight placement (trim, belt, harness, ankle)
Average depth for SAC calculation
Did my pony come along
Room for what we saw, found, collected
Where we ate afterwards!

I'm surprised you did not get to nitrox with those fields yet.

That's a nice form and again I didn't mean to take anything away from what you have there.

why bluetooth? why not directly from the cloud?

Not sure we're on the same page....my original suggestion was to download from the dive computer via BT to an Android phone. Seeing that there is only one company to my knowledge making BT-enabled dive computers (Shearwater Research), adding the ability to download from the dive computer via USB to the phone would also be beneficial.
