It sounds like he removed his reg on the surface, choked on water, and incurred a laryngospasm maybe, or it could have been a cardiac event. I had one of those once and thought I would drown in the pool's shallow end. I worry about divers who spit their regs out in waves on the surface. I keep mine in until I am out of the water.

A diver has died after an accident in an indoor swimming pool in Swabia. Several members of a diving club were in the pool in Immenstadt im Allgäu (Oberallgäu district) on Sunday when the 56-year-old man swallowed water when surfacing and then had to be resuscitated.

I usually switch to my snorkel on the surface, immediately. If you do that, especially with the snorkels now that they don’t really allow water into the mouth of the diver, this could be prevented too.
