Thanks Baitedstorm :) I kept it simple for the OW students since they make them before their check-out dives and one of the dive sites we use has a hospital that doesn't readily show up on the search engines, talk about a PITA. Most of them end up doing the assignment twice before they get the right hospital and a map from the quarry to that hospital. The easiest way to get a map is to find your dive site on a USGS topo map then find the closest hospital sign, copy the part you need and highlight the route.

I got really lucky, my dive site is right next to the hospital, within a few miles. I usually only show the underwater map to the students, and not all the time. I thought it might lessen some of the stress of what "might" be done there. I definetly like things more simple, for my own sake. I'd like to include your format to my own personal dive log if you don't mind.

For Every dive site a planner is made that contains.....


Dive Site:

Drive Time: x Hours

Dive Type: (includes level / difficulty of diving, whether a lake, quarry or wreck dive,
any special requirements needed)

Lodging:

Meeting Location:

Facilities (Example: Camping available with swimming beach.
& Fees Entry fee:$8/diver/day Pit toilets, changing room. Concession area
serves pop, chips and grilled food.)

Equipment needed: (Example: One full tank, warm and dry clothes, snacks and
beverages, dive light.

Environment: (Example: Water is cold, especially below thermal cline. Full thermal
protection is recommended. Mud over rock bottom, easily mucked-
up. Most sites are at 25’ to 50’. Fairly easy to navigate. Anchored
platforms for checkouts. Fish, boats, vehicles and structures.)

Nearest Dive Shop: (include address, phone, available services)

Reverse side of planner has directions and map to the charter launch or quarry. & an underwater map of the lake or quarry.
