Bear in mind that most deaths on dive boats (or around any diving activity) have nothing to do with diving - they're from heart attacks... so an AED is nice if you can have one available.
In addition to the stuff already mentioned -
Plenty of oxygen
Enough bandage material to construct a large pressure bandage
EpiPens
I like to try to book at least two ER/EMT folks on the trip :)
A doc is nice to cover the legal aspects of carrying EpiPens (and morphine, for that matter).

I fairly new to diving - I've only been certified since June... This may be a bad question, but what about carrying a pony bottle onboard with O2?

No mask strap cause I bring a spare mask

A few hundred O-rings of all different sizes

Spare computer batteries

Regulator HP and LP port plugs
Back plate screws
Spare inflator and coupler
Signal mirror
Bio seal for dry suit neck seal

1/8 Vacume line used for dry gloves
