I have to agree that having a student die during training is not an acceptable outcome, baring the strangest of occurrences.

... and in the absense of information, nobody knows what circumstances led to this diver's death.
Blaming the instructor at this point is not only irrelevent, but a violation of the rules for posting in this forum.
