Does anyone know if the LDS sends those forms to PADI to be logged in a system, or are they just for the LDS's personal use?
 
For all recreational courses remain at the LDs for a period of no less than 7 years on your file.

This depends on the local laws but 7 years is about average
 
What if you cancel the course before you start, do they still keep a file on you?
 
If you filled out a student record file and handed over the liability and medical papers, all will be kept, you can expect a call from the LDS at some stage asking if you are still interested in doing your course.
