This sounds like a skills problem not a gear/set-up problem. The person handing off the reg should be making sure that it is right-side up for the recipient.
If the person who is OOA is taking a clipped off reg then it is their responsibility to deal with orientation.

Or should be using an ambidextrous second stage... Like the posiedon or hollis dc7...
