Well this isn't a pet peeve, which would suggest that it happens from time to time. More of a peeve because it only happened once.
When I was doing my OW cert 5 years ago I was to swim around the pool with a partner(who was to guide me) with no mask on. After the instructor signaled for me to take my mask off, my dimwitted partner(who was 12, I might add he didn't do hardly any of his bookwork, and is just all around not a very attentive person) did not grab my BCD and guide me like he was directed to. Rather, he just floated stupidly, staring into space, while the instructor had moved on to instruct someone else, and never guided me. I ended up putting my mask on, clearing it, the DM shook his BCD to signal his to take his off(my partner STILL not paying attention, having only removed it when he came to at his convenience) and I had to guide him around the pool.
After the skill was completed and we had surfaced, the DM asked him why he had not payed attention, told him the consequences of such act, etc. His response was: "I was looking at the others throwing the little torpedo thing."
The others had already completed the skill and the other instructor was working with them on neutral buoyancy, throwing the torpedo to and from them to demonstrate neutral buoyancy
