It might be helpful when you post a news video that is over a half hour long to state exactly where in the video that specific bit of news is actually located.

Condolences to her family & friends.

I guess in panic situation she didn’t get a chance to get a hold of the 2nd stage / octopus & open the air valve to breathe through the regulator when she went underwater.

First thing that struck me...the guide leading the dive was a “tourist” DM, so while he dove the site before, he was not local.

Rocks and waves...she could’ve been knocked unconscious before she could do anything.

I wonder how experienced the victim was. Didn’t say.
 
Sad. The image I have in my head of watching this unfold from the description, seems like plenty of room for improvement. A 62 yo woman, on a wet rock, with rough water, and I'm going to out on a limb and guess her reg was not in her mouth. Wow.
