I work at a shop and had 2 discs blow today. The valves are less than 1 year old. 1 blew at 1800psi and the other at 3400. It is the PVD Pro Valve #VX2PVD-34N-VI Any one else had any disc problems?

My friend just returned from dive trip and her New Sea & Sea camera(Ricoh) stopped working on the second dive. The internal strobe failed. After taking it out of the nice housing, the bezel around the lens fell apart! Not off to a good start. Has anyone else had this problem?
