Probably due to their rarity. Most people having bad enough breathing issues to cause such an injury probably won't be diving anyway.

You're probably right--that's what I was thinking. The instructor (CD) who mentioned it said about the only time she wouldn't dive would be with any serious chest congestion. I figured we all get that at some time or another.
