Leech. Looks like the base of the extended proboscis (feeding organ) at the tip of the head on the left.

That was my first thought as well.
Ever heard of a leech attaching to the eyeball of a fish?
The only reason I thought copeopod was there appear to be two rounded appendages as well, toward the right side of the body.
