You likely could not have prevented this tragedy.

When I was getting my DM certification years ago, there was a man that was trying to get rescue certified with the same instructor that I was working with. He was morbidly obese and one wonders how he was cleared to be certified in the first place. My instructor told him he was in no condition to do the physical requirements for the course and that he needed to see to his general fitness first and that would be his first self-rescue.

The candidate was irate and took his business elsewhere rather than take the very good advice that he got.

Hindsight is 20/20 and "Steve" is more likely to have ignored your advice rather than taken it. Your response was commendable.
