Dive Report Help

Quick question. A group of friends and I are travelling to a new dive site that most of our LDS instructors have never dove. They have asked us to write a dive report about the area. What are some common things that we should include in the report?

I'd say the same things that you wish you knew already.
Access, water temp, vis, depth, current, type of dive (cave, quarry, open water, fresh/salt...), what there is to see, local dive ops, hotels, restaurants, rules specific to that site (i.e. the Coz 'no knives' rule)...

Trip Logisitics: How do you get there? Where do you stay/eat? Who are the dive ops? Costs? Rental equipment?

Dive Site(s): Name? What is the typical dive profile? What was the entry like? Viz? Currents? Any special considerations or risks? What did you see? What did you like about the dive?
