Does anyone here have valid statistics for diving related injuries and/or deaths in the United States?

Specifically, I'd be interested in knowing:

1. How many people on average die each year while diving in the US?

2. Of those, is it verifiable how many died due to a heart attack (MI), as opposed to DCI or AGE?

3. What are the total number of dives done each year in the United States?

4. What are the total number of serious diving injuries, such as DCI (or DCS), AGE, etc.?

Any info or partial info appreciated.
 
DAN publishes an evaluation of the reported accidents in the US every year. If you're a member you can download them for free.
