DIY Dive log books - any ideas?

Hi folks,

any ideas on how to make your own dive logs - any good layouts that you use / print out at home, whatever?

Any web links?

I am trying to find a log that will fit into a Personal Organiser that is a 6 ring binder... any ideas?

I use this and put them in a three ring binder. The image could be reduced in size for use in a personal organizer.

Based on input from SB I made my own in word and printed on water resistant paper and got a cheap plastic 6 hole punch that matched the fancy leather organizer that was a cast off. Works real nice.
