What yoke/regulator is this happening with so much?
I'm guessing its a unique mismatch between those yokes and their latest batch of valve donuts.

Or poor quality control when machining the o-ring groove.

I had the same issue and Blue Steel replaced the insert. Said their could have been a milling error.
 
Its on a tank whip in a fill station that its happening to me. But Its also happening on customers regulators as well.
