Here's what I use:

Hoodie Lens Caps - LensCoat

Works well. I can tether it to the handle, too, so it basically just tucks back while I'm diving. I put it back on before I get on the boat.
 
What size do you use and on what port?
 
Hi -

I use a SMALL on my Flat Port 72 (the 18-55mm). It's 3.75" across. On the Macro Port 45, I use an X-SMALL. It's 3" in diameter. Hope that helps!

I have the same port but I measured it to be 4" across. Is the small tight ? Do you think the medium would work better ?
 
You could definitely try it! I was lucky enough that my camera shop (Samy's) carried these and I was able to take the lens in. The neoprene is pretty stretchy. I'm wondering if a local shop might carry these or something very close that you could try?

Can you describe how you tether it to the handle or post a pic ? does the opening taper at all ? I'm worried the med will be too loose since the front of the port is 4 inches but is smaller a little beyond the front glass. I'll have to see if a local camera shop has these or not.

Not a Nauticam, but I got the idea from a custom DIY project for a Nauticam.

It's an Op/Tech "Lens Hat," folded inward and glued (for perfect fit and stiffness) with a strip of nylon webbing sewn on and some bungee.
