That 'shaded portion' you refer to is the diffuser panel over the flash. It disperses the light, spreading it out for wider coverage and preventing harsh shadows.

So the particulate in the water does some work for you this way. I guess that with ext strobe the offset from the lens has the same effect??

No. Particulate matter doesn't diffuse light. It gets 'highlighted' by the internal flash or strobe, causing it to show in your photos. It's what we call backscatter. It's worse on fixed strobes and internal flashes that throw the flash straight out from the camera because the reflection off the particulate matter comes straight back to the camera. When you have an external strobe on an articulating arm, it can be aimed at an angle causing the reflection on an angle and as such it doesn't get highlighted as bad. Does that make sense?

Think of a triangle. You subject is at point A at the top. The base of the triangle is your camera/film plane...ex. B_____C. Your strobe is at point B, angled toward point A at the top and the light is bounced back to point C. If there's enough particulate 'stuff' in the water, you'll have backscatter regardless, it just won't be as bad.
