Ok - I'm not an instructor - but from what I've seen -

*trust me dives
*inattentiveness/non-understanding of gauge/computer readouts
*inadequate/no dive planning
*not adhering to the plan
*complacency
*non - realization that you have your life in your own hands - and shouldn't depend on someone else to do it all for you.
*pigheaded stubborness

That's my list - but I'm no instructor!
