I can relate to your wife's needing to descend feet first .... I, and may divers, can equalize much more quickly in a "feet down" position. However, my weight distribution is such that I can pivot from "head up" to "head down" and 'horizontal" with no effort. Takes some time (and varies with BCD, wetsuit, and fins) but once I get it right I note the total weighting and where they are placed in my logbook so I can repeat the config easily. In my case, I'm a bit "feet heavy" so I wear more weight on my tank bands and less in my BCD pockets.
