Another one bites the dust ...
 
Here's a link to an extensive report of the disaster based on a debrief of three of the surviving divers. It ain't pretty. Poor briefing, uncharged extinguishers, the vessel underway and speeding up, fanning the flames and hampering evacuation to the Zodiacs, and more. They didn't know anyone was missing because there was one more crew member aboard than the divers were advised of, so that the head count appeared complete when it wasn't. The writer says, "[w]hen we hear about everything that went wrong with the Dive Pro Liveaboard-operated vessel during the accident and rescue, we are speechless." Nauseated is more like it.
