Can someone please point me to the DAN? BSAC? incident report which states that the majority of diving incidents occurs on the first day of the vacation? I have been searching but can't find the thread where we were discussing this.

Someone here posted that 85% happen on first day or something like that, but I don't know the source. Hope someone does.
 
I don't have the energy to go search the DAN site tonight, but it's in one of the DAN incident/accident yearly summaries.

Of the total number of dive fatalities analysed, 88 per cent were on the first dive of their vacation or trip. Looking beyond these profiles, the research examined the root cause or “trigger” that set off a series of events that resulted in a dive turning bad, and in these cases ending in death.

Well, that seems to be how the rumor started, and it may be true - but verification would be nice.

I have been the source of some of those postings. A couple of years ago a representative of DAN did a presentation at a social gathering at our dive shop. She said that over 80% of DCS accidents happened on the first day of a dive trip, and over 80% happened on the first dive of that day. I have repeated that information several times on ScubaBoard. Unfortunately, there is no way to link to her presentation.
