Attached photos of my reg, which I haven't used for a looong time. I'm hoping to get back in the water and want to see if parts/service kits are still available for 2nd stage.

Could anyone identify which model this is?

You can change the title. Click on "thread tools" at the top. Then select "edit title"
 
R109. The LP hose fitting for the R108, IIRC, is angled back and does not have an adjustment knob.

Yes, you can have the second stage serviced. Whatever you do, don't damage the rubber diaphragm cover. It isn't made anymore and can be very hard to obtain. Keep it rinsed, in good shape and don't let it dry out.
 
Extremely simple to rebuild, easier if it already has the Duro poppet or the balance adjustable (156) conversion, all of the soft parts are available from VDH.

Looks to be a 156 to me not a 109, assuming knob and rubber diaphragm cover are original. All parts readily available. Simple to service. If you have any issues with parts message me but as suggested VDH will have all you need. Good luck, they are great regulators.
 
Before you buy parts, determine which poppet you have and if it has been converted/is a 156. The kits are different.
