What is the cause of death for most commercial divers? Who is at fault most of the time, the company or the diver?
 
I wonder if those guys get to go to court for negligent homicide
 
More often than not the ultimate blame lies on maintenance.

Sometimes it is the company equipment and sometimes the divers personal gear.

Using the wrong gear, incorrect gear, or not having adequate experience in the use of the gear.

I can not recall a negligent homicide case but there have been many insurance (Jones Act cases) and civil suits brought against the dive companies, their insurance companies, in several cases the customer that hired an uninsured dive company.

OSHA requires the supervisor on a commercial diving project to have a letter from a company officer designating them as the person in charge, so ultimately the supervisor is in many cases responsible but company has deeper pockets.
