Good Phuket dive live on board dive operations?

I will be on Phuket December 11-18, and plan to dive good sites in Andaman Sea. Three-four days live on board will be good. Can anybody recommend a good dive operation to go with? The one that (i) go to right places with experienced divers on board, (ii) not a crowded, 10-20 people on the boat, (iii) comfortable accommodation on board, I can share a two-person cabin, shower, head in cabin, good cook on board is appreciated, (iv) reasonably priced?

There are a few options for you, but boats are getting full for that time. Take into account that the boats with a bathroom en-suite will in general take more than 20 people. The boats are big so it wont feel crowded though.
