When flying to a destination like Bonaire, the plane is at the least a Boeing 737 from Miami..... so I just put a shitload of the "really heavy" items into my NorthFace backpack as my "personal item" and there is no issue. I'll bet that my basic backpack slung over my shoulder weighs 30lbs......but they don't care because it's basically my "purse".

Also..... and this will probably be unpopular for sure....... but you can even bring an "additional" carry-on if it is identified as a medical device like a CPAP.
