The New Zealand police national dive squad reported on 39 diver deaths that they investigated between December 2006 and March 2016. Nine of these deaths were recent and a big jump in the statistics. Read the report here: Police National Dive Squad – Water Safety Presentation | Dive HQ New Zealand.

Summary:
85% failed to follow buddy system or got separated
33% failed to monitor air adequately
54% of cases their dive fitness was questioned
46% were overweighted
90% still had their weight belts attached (or removed by their buddy)
49% had unserviced gear or used gear with known faults
 
I don't see a stat for how many had CO in their tanks....
