I only know of Goodenough Spring (Lake Amistad), but it is an advanced cave dive and it starts around 140-150' depending on lake level:

Jacob's Well is permit only. Phantom Springs is also permit only.
I have "heard" of others, but they are on private land with no access to divers. Quotes in this instance means...please take with a grain of salt, it's just hearsay.
