The East and West walls out of Salt River have great diving when the surf is reasonable and only a 5 minute boat ride with Anchor Dive. I've always had good diving weather there in July.
Coki Beach Dive center is nearby, good for a quick shore dive, looks like they have a small boat now too. Red Hook Dive Center for a bigger boat. Haven't been there since Chris Sawyer closed. Sugar Bay resort has nice views, beach, and pools.

On typical warm water day boat dives in Florida, Carribean, Hawaii with DM we usually tip $5.00 a tank for the two of us for the DM/crew. Captains are typically salary and say they don't get tips but the "crew" appreciates it.
I dove with an op on KONA that had powdered soup and hot chocolate packets along with thermos of hot water. Both of these worked well for warm up. OK, so water wasn't in the 40s but for night dives with 3/4mm wet suit it was still a big help.
