Be sure to have this info ready...
*Full name
*Contact phone number
*E-Mail address
*Agency & Instructor number
3) You must also add the correct information to the "Agency/Level/ID number" field in your user profile.
And click the PM to Dee in blue there to intiate a PM to the lady who handles this.
If you run into problems, PM me (see below) and tell me about it...
