I would also recommend doing an O2 class. I took the DAN O2 class and it was pretty good, it did not feel like I was just paying money for the card, there was real substance to the class.
 
Of the courses I've taken Rescue was one of the most beneficial. As for the EFR it depends highly on the instructor.

I was lucky and got ine with an ENT doctor but I've also checked out other courses through dive shops and some appeared very weak.

I've also gone the St. Johns route and have taken some pretty thorough courses.

If I wanted a solid general course I would not go with a dive shop unless the instructor had specialized medical training.

The rescue course I took also included a large section of first aid so the source of your cert. should not really matter in the end.
