I see my doctor every six months and I have him sign the SSI medical release form (PADI has a similar form). I make multiple copies and give a copy to the dive op I'm diving with. The form has never been turned down and I have peace of mind that all bases are covered.

Sorry, if this advice has already been given. Didn't want to wade through all the responses before replying.
 
Ironic I'm reading this thread now as I'm waiting for my doctor appt for my annual physical, where I'll have the doc sign off on my medical form (allergies requiring daily OTC medication).

Sorry, I've read it three times and do not understand your post. Can you restate it in different words? Maybe with fewer piled on negatives?

The form I have is two pages. The first page you refer to I fill out and sign. The second page, at the bottom, has the doctor info and he signs that section. In that section is the question, "I recommend this individual is fit for diving" and a yes or no check box (or something similar to this statement. Here is the form I use:

Look at the form. Page one has the questionnaire and your signature and date. Page two does not have your signature anywhere. If you are not submitting the questionnaire, they don't get your signature. If they're taking you without your signature, they are full of crap.

PADI does not agree. Page 1 is unnecessary if you provide a signed page 2.

Interesting. So you don't have to give them a signed "I agree to accept responsibility for omissions" statement? I wonder if their lawyers know...

PADI says the purpose of page 1 is so the instructor can decide if page 2 is needed. If it is needed, it is all that is needed.
I suspect the purpose of the page 1 statement that you are obsessed with ( :)) is to cover the instructor when he/she gets a questionnaire that is all NOs, in which case page 2 is not needed.
 
How has this medical statement ever protected the LDS anyway? If the family sues, it is not because the guy had a heart attack, but because the DM or instructor "abandoned them" after they have the heart attack.
When the DM or instructor is dangerous, incompetent or crazy, they do get sued, as there is no release signed against that situation, so they are covering their butts, but not their nuts.
