Incident involving death of an adult instructor and a 13 year near Naples Italy

The main newspaper of Napoli Italy, Il mattino reports that this morning two divers went missing during a training dive between the island of Ischia and Procida.
The adult diver is the owner and operator of the diving center Sea World Mr Antonio Emanato his body has been recovered.
The youger diver, officially missing, is Lara Scamardella. Currently the operation is ran as a body recovery. She is considered experienced. No mention of her level of certification if any, while il corriere della sera, main italian newpaper, reports that this was the last dive of the certification.
There are no indication about the causes.
Athird source (linked above) tgcom24 instead reports that the divers entered in a small cave at 10 mt and were unable to egress. They were immediatly located due to the smb being tied outside the cave and only one body was recovered because of the silt that was making unsafe for non cave rescuer to attempt recovery.

I am an Italian certified cave diver, I am apalled by the superficiality with which expert divers, instructors, lead less experienced divers to their doom especially in my country this is at least the third episode in the same number of years.

Very sad to see it happen, particularly to a 13 year old child. Please do stay away from caves if not certified or lead by a certified instructor during a cave course. Please avoid trust me dives. I am guilty, I did a trust me dive just prior to start my cave training ... only after training I fully understood the risks involved. These incidents are a clear warning to anybody willing to venture beyond the limits of their training.
 
This is just so sad. Unfortunately it's not the first time this has happend in these sea caves/caverns and probably won't be the last. :(
 
I have never met a 13 year old that I thought was mature enough or responsible enough to be diving in a cave. Period. So sad that an instructor would be so foolish with the life of a young student. Such a waste.
 
They went into a cave. Without training or equipment. So no guideline and the minimal skills or a kid on her 4th ever dive stirs up silt. Now you have no visibility. And this was probably done after the dive was over, so they don't have a lot of gas. They certainly don't have the 200 plus cubic feet they should have. So they die a horrible death when they run out of air while desperately trying to find the exit. And it sucks and is is totally the fault of the instructor.

This makes something like 10 people killed in Italy this year after their guide decided to take them into a cave without training or equipment.

Do not go into caves without proper training and properly equipped. No matter how nice or inviting it looks or how much your wonderful guide encourages you to go in.
