As was mentioned he did not remove his weights.

Disappointing but not surprising. "Surfaced then went back under" is very high on the list of reasons for fatalities.

Somehow, weight ditching at the surface needs to become a much more frequently practised skill.

or unable, to ditch their weights when they get into trouble. It is likely that, on
many occasions, by the time divers recognize the need to ditch their weights, they
are too incapacitated to do so."
