Situational awareness is key as several have pointed. I had a situation where I did not check the isolator pre dive and started the dive with it closed. During the dive I noticed my air consumption much higher than normal even though I was not working hard. Really bad mistake on my part. But right in the beginning phases of the dive, I knew there was something wrong. Long story short, I opened the isolator UW and saw my tank pressures go up on the perdix.

Bad mistake. I won't make that one again.
