Still cant see how that is any more of a failiure risk than existing DIN threads or existing A-clamp threads. All the adaptor is, is a piece of metal with a 7 notch DIN connector standard (just as you get on a tank) and an A-clamp (just as you get on those connectors). Its not a new failiure point in any shape or form - its simple what youd find on the tank anyway.

Of course any dive kit can be mistreated to the point of being unsafe, regs are no different, tanks are no different but that isnt really the fault of anything.

Ive seen a A-clamp spectaculary let go (as in the metal connector actually cracked and went bang) which again i suspect was due to taking a hefty knock at some point.

Ive not seen a tank for years that wasnt DIN convertable (by simply removing the insert) in any case. Certainly not in europe or the UK.
