Please help with a diver survey

Done. Looked like the entity the survey is designed to inform is a tech. diving-related one, but this is posted in Basic, and some answer options reflect recreational divers, so I figure that's legit.

Interestingly, the focus group questions ask for my town and my country, but not my state. Seems like knowing my state would be more meaningful? After all, across the U.S. a lot of towns may share a name. Unless I read it wrong.

One comment: The travel distance options were clearly US-centric; the alternatives weren't particularly fitting for e.g. European participants.

Always aware of the limitations when pushing out a survey to an international group. It truly is a challenge in some instances... especially when costing stuff (Euros, Dollars US, Dollars CAN, Pounds Sterling!???); however, Across State/County/Province seems to cover the US, Australia, UK and Canada.

Richard, customer really is not interested at this time in the state, county, department or whatever. At some later point, they may want to refine data a little more, but for now, town and country is fine.

Done. Good luck. My year has been crazy so some of the answers are likely a little weird. I tried to answer what was normal for me.
 
Done. For travel I checked chain motel, but that's for driving down to the Gulf. I dive the 3 months there from our rented condo.
