I want my 2nd stage regulator to go under my right armpit and up. how long should the hose be? I assume i'll need a 90 degree swiven on the 1st stage.. what about on my 2nd stage? same 90? or a 45?

Lance I would recommend looking at the Dive Rite Videos on YouTube which show how to set up a open water regulator.
 
I use a 40" primary with a 70 degree adaptor from DGX. I also have some Kirby Morgan ones which are a little better made, and more expensive. I find the 70 a tad more comfortable than the 90.
 
I used to use 110 (or 70 - same thing) degree swivels on my primary 2nd stages. I recently changed them all for 360 degree swivels from Cave Adventurers.

I definitely like the CA swivel better than the fixed angle swivel.

I do not currently have a swivel on my 1st stage, but I am planning to order one from Piranha and add it to my 1st stage.

First Stage LP 360 Swivel

The hose for the primary 2nd stage coming straight out the side of the 1st stage is not really a problem. But, it does tend to make that hose want to float out to my side, instead of staying neatly tucked in against my side. Sometimes, depending on what I'm doing with my right arm, that hose can end up outside my elbow and I have to reach my elbow around and tuck it back in. And that is even with me having a MiFlex hose, which is more bendy. If I had a rubber hose (which I will change to when the current hose needs to be replaced), I think it would have even more tendency to float out to my side.

The downside to adding the swivels is that they add extra O-rings to your regulator setup. That means more potential points of failure. It's a tradeoff and you have to decide for yourself if it's worth it to you.

yes, this is my concern.. having hose floating outwards. Hence considering swivels. still considering the 70/110 over the 360 swivel... so far. i've had fellow divers use the 70/110 swivels and the 360. 360 tends to swing around while off the boat. I have to consider also the gear off the boat. some boatmen just drag the gear!

yes considering the 70 degrees. is the 70 in your primary 2nd stage?
what about your 1st stage? no more swivel?
 
Yes, as per post, I use a 70/110 on my primary second stage on a 40" hose. Never used or felt the need to use a swivel on my first stages.

I don't let anyone touch my gear, ever. It's a polite conversation I always have when getting on a new boat/liveaboard.

alright! might as well do that.!

so from the first stage.. it goes out a bit (because of the wing) then goes under the arm up the mouth? this is correct right?

i use a scubapro xtec single tank plate and donut wing. 5'6" height athletic build.
 
FWIW I've tried the 360 swivels from CA, the weight was noticeable to me and I didn't find them anymore confortable than the 70 so sold them. YMMV.
